Verbal memory and sex: patterns from childhood to later life

Verbal memory encompasses the encoding, storage, and retrieval of language-based information, including word lists, narrative passages, and conversational content. Across the lifespan, performance on tasks such as list learning, story recall, and semantic fluency shows considerable variability, and a substantial body of research has examined whether males and females differ systematically in these abilities. While meta-analyses often report small to moderate female advantages in some verbal memory paradigms, the magnitude and direction of these effects shift with age, educational exposure, and the specific demands of the assessment.

Understanding these patterns matters for clinical neuropsychology because misinterpreting group-level differences can lead to diagnostic error. A score that appears borderline for one sex may be well within normal limits for another, and vice versa. In Australia, where neuropsychologists registered with AHPRA routinely assess individuals from diverse linguistic and cultural backgrounds, careful attention to normative standards is especially important. The Australian Neuropsychological Society has long emphasised the need for demographically appropriate norms, including those that account for sex- and age-related variation.

Studying sex differences is also methodologically demanding. Hormonal fluctuations, reproductive history, medication use, and sociocultural factors all interact in complex ways that are difficult to disentangle in cross-sectional designs. Longitudinal cohorts, such as those maintained by the Australian Imaging, Biomarkers and Lifestyle flagship study, have begun to shed light on how trajectories diverge between men and women over decades of follow-up.

This article synthesises current knowledge about how verbal memory differs between males and females from early childhood through older adulthood. It draws on international literature while highlighting findings relevant to Australian clinical practice and the country's multicultural population.

Early development and childhood patterns

In preschool and early primary years, girls often outperform boys on tasks involving rapid naming, phonological awareness, and the recall of verbally presented stories. These differences tend to be modest and are influenced heavily by language exposure at home and the quality of early childhood education. Australian data from the Longitudinal Study of Australian Children have shown that girls entering school in Sydney, Melbourne, and Adelaide tend to have slightly stronger vocabularies than boys, although the gap narrows once socioeconomic factors are accounted for.

Boys, in turn, sometimes show relative strengths in spatial components that support certain memory strategies, though these advantages do not consistently translate into superior verbal recall. The interaction between spatial and verbal systems is relevant for interpreting performance on list-learning tasks, where approach strategies can influence outcomes independently of pure verbal ability.

Pubertal and adolescent shifts

The onset of puberty introduces hormonal changes that appear to influence verbal memory consolidation, particularly through oestrogen's effects on hippocampal function. Some studies report that female advantages in verbal episodic memory become more pronounced during late adolescence, while others find the differences stabilise. These findings are complicated by the fact that educational experiences and reading habits also diverge during the teenage years, particularly in contexts where girls are more likely to engage in extended literacy activities.

In Australian secondary schools, where the curriculum emphasises both STEM and humanities, adolescent reading patterns vary widely. Researchers at the University of Queensland and Macquarie University have noted that participation in debating, drama, and English literature correlates with stronger verbal recall performance in both sexes, suggesting that environmental enrichment can modulate group-level differences.

Hormone replacement therapy may attenuate some declines in verbal memory when initiated around the time of menopause, but findings are inconsistent and individual responses vary.

In older age, both sexes show declines in episodic memory, but the trajectories differ. Australian research conducted through the Centre for Healthy Brain Ageing at UNSW has suggested that women often maintain stronger verbal memory into their seventies, whereas men's verbal memory scores tend to converge with women's in the eighth decade. These patterns have implications for the early detection of dementia, particularly Alzheimer's disease, where verbal memory impairment is a hallmark feature.

Clinical and neuropsychological implications

For practising neuropsychologists in Australia, the practical message is to use sex-specific normative data wherever possible and to interpret borderline scores cautiously. The range of assessments used, including the Rey Auditory Verbal Learning Test, the California Verbal Learning Test, and the Hopkins Verbal Learning Test, each comes with normative samples that vary in their representativeness for Australian populations.

Broader neurological research also underscores how sex intersects with other cognitive and sensory domains. For instance, work on olfactory changes in Parkinson's demonstrates that women and men may experience prodromal cognitive changes differently, reminding clinicians that sex-sensitive interpretation extends well beyond memory alone.

Cultural and linguistic considerations in Australian practice

Clinicians working in remote communities, whether in the Northern Territory or Western Australia, must adapt their approach to ensure that assessments are both culturally safe and clinically meaningful. Recognising these layers of diversity helps avoid simplistic interpretations of sex differences in verbal memory and supports more equitable neuropsychological care across the country.
